| Procedures |

In the event of an incident likely to give rise to a claim under your insurance policy, please take the following steps:
| Motor Insurance |
|

Report the accident to the Police within 24 hours. |
|
• |
Notify us of the accident by calling our Customer Service at 03-2034 9888 or our nearest branch office. We will post to you the relevant accident report form.
Alternatively, you may personally visit our Customer Service
Centre at our Head Office or at our nearest branch to collect the accident report form.

Documents required for processing of a claim:
- Motor Accident Repair Claims
- Select any registered PIAM workshop to carry out the repairs to the car; they will prepare a Repair Estimate for our attention. Most workshops will also assist the insured to submit the claim documents electronically for our processing. If necessary, we will assign an adjuster to inspect the damaged car.
- Accident Report Form.
- Police Report.
- Photostat copy of the vehicle registration card (front & back).
- Photostat copy of the insured’s and driver’s driving licence and identity card.

Motor Windscreen Damage Claim
- Windscreen/Window Glass Damage Claim Form.
- Police Report.
- Original bill for the replacement of the windscreen/window glass.
- Photographs showing the damaged windscreen/window glass, one of which must clearly show the vehicle number place, road tax disc and the manufacturer’s markings on the glass.
- Photostat copy of the vehicle registration card (front & back).
- Photostat copy of the insured’s and driver’s driving licence and identity card.
- If there is a claim for the sun tint film, documentary proof the vehicle was fitted with sun tint film prior to the accident.

Motor Theft Claim
- Motor Theft Report Form.
- Original vehicle registration card, submit on settlement of claim.
- Original certificate of Insurance.
- Police Report.
- Full set of keys.
- Hire Purchase Agreement, if any.
- Hire Purchase Release Letter, submit on settlement of claim.
- Photostat copy of the insured’s and driver’s driving licence and identity card.
- JPJ Forms K3 and K3A (2 sets).
- JPJ Form TM-AB (Letter of Indemnity) duly signed and witnessed.
- Past 3 months salary slips of the insured and driver.
- Latest income tax assessment notice (Form J) of the insured and driver.
- Letter from the police on the outcome of their investigation.
- If the insured is a coporate body:
- Photostat copy of certificate of incorporation of the company or Registration of Business.
- Returns to the Registrar of Companies, or Form 9, 13, 24, 44, and 49.
- Annual accounts for the 3 years.
- Photostat identity card of the official who signed the JPJ Forms K3, 3A and TM-AB.
- Company Resolution, submit on settlement of claim.

Do not negotiate or admit liability to the claim made by the third party without our consent.
